If you or a loved one would like to inquire about selecting Ottawa Grace Manor as a home for your loved one, please contact the Champlain Community Care Access Centre (CCAC) at 1-800-538-0520, or visit their website: healthcareathome.ca/champlain/en. All applications to long-term care homes are coordinated by the CCAC. The CCAC can provide more detailed information about how to apply for and select a home, and also how to prepare for the move.
